We appreciate your interest in acquiring a Rusticwood puppy.
Please take a few minutes to fill out our "Puppy Questionnaire"
to help us determine if a Rusticwood puppy is right for you.

You must download the questionnaire to your computer to be able to fill in the form.

Downloading the form can be accomplished by "right clicking the form" once it is open and selecting "Save Page As..."
Your computer will prompt you to select a location on your computer to save the Puppy Questionnaire.

Once you have downloaded it, you should be able to type your answers in the spaces provided.
After you have completed the questionnaire, save it to your computer.

Please send an email to Rusticwd@aol.com and attach your completed questionnaire to the email as an attachment.

If you experience any difficulty whatsoever, please email us and we may have an alternative to the questionnaire.
